First visit:

Please bring the following to your first Provision Physical Therapy evaluation.

• Identification

• Insurance card

• Comfortable fitting clothes to move around freely

 

Expect your visit duration to be at least forty to sixty minutes. Each evaluation is personalized to each patient’s needs.

Cancellation and No shows- All patients are expected to give a 24-hour notice prior to their appointment. Fees may apply for cancellation and no shows less than 24-hour notice.
